1. Where are you located?
We are conveniently located in South Fort Myers at 12995 S. Cleveland Avenue, Suite 184.  Our office is north of the Bell Tower Mall, just east of the Outback Steakhouse, across from the Crowne Plaza Hotel.  See the map under Directions on our home page.

2. Do you have evening hours?
Yes... Except for major holidays.  Our office is open
Monday - Thursday from 7:30 a.m. until 7:30 p.m.
Friday  8:00 a.m. until 6:30 p.m.
Saturday 10:00 a.m. until 3:00 p.m.

3. Do you do school physicals and how much do they cost?
Yes, the base charge is $44.00.  There could be additional fees depending on what else is needed.

4. If I get sick and I need to be admitted to the hospital, do your doctors have admitting privileges to the local hospitals?
Our doctors have been credentialed and are on staff at local hospitals, but have chosen not to do inpatient care.  If you should require admission, an appropriate specialist will admit and care for you.

5. What insurances do you accept?
The list of insurances that we accept is too large to list here.  We do take most insurances, but it is best to call to verify if we accept yours when you set up your appointment.

6. What credit cards do you accept?
We accept Visa, Master Card, American Express, and Discover.

7. Can nurse practitioners write presecriptions?
Yes, except for narcotics.

8. What is a Nurse Practitioner?
Nurse Practitioners (NP's) are registered nurses who are prepared, through advanced education and clinical training, to provide a wide range of preventative and acute health care services to individuals of all ages.  NP's complete graduate-level education preparation that leads to a master's degree.  NP's take health histories and provide complete physical examinations; diagnose and treat many common acute and chronic problems; interpret laboratory results and x-rays; prescribe and manage medications and other therapies; provide health teaching and supportive counseling with an emphasis on prevention of illness and health maintenance; and refer patients to other health professionals as needed.

9. Do workmans comp patients need appointments?
No, they may walk-in at any time during our normal business hours.  They do need authorization from their employer or a workmans comp insurance carrier to be seen.

10. Do you perform drug screens and/or B.A.T. (breath alcohol testing?)
Yes.

11. Do people coming in for drug screens need appointments?
No, they may walk-in at any time we are open.

12. Do you perform D.O.T. (Department of Transportation) physicals?
Yes.

13. Do you perform x-rays?
Yes, if it is allowed by your insurance company.  Some insurance companies have arrangements with specific x-ray facilities that they require you to use.

14. Who do I call to get my prescription refilled?
We have a 24 hour message line that you can call to request a refill on presecriptions. 
The Prescripton refill phone number is:  (239) 333-1768.  Please be sure to leave your name, date of birth, your telephone number, the pharmacy name and phone number you wish us to call.  Also include the name and dose of the medication you need.  Please understand that this process may take at least one business day and some medications may require an office visit before they can be refilled.

15. Do you prescribe the "Morning After Pill". 
No.

16. Do you do laboratory tests?
We have the ability to draw blood in our office that is ordered by one of our providers.  It is then transported to a lab for processing.  However, some insurance companies require blood to be drawn and processed at a specific lab.  We do have the ability to perform certain testing on site such as: pregnancy, flu, strep, urinalysis, PPD and PT/INR.

17. Do you do immunizations?
We have the following immunizations available:
            * Tetanus, including Dtap, Tdap, and Td
            * Hepatitis A and B
            * Influenza-for flu
            * MMR- Measles, Mumps & Rubella
            * Pneumovax- for pneumonia
            * Gardasil-for HPV (Human Papiloma Virus)
            * Zostavax- for shingles
            * Menactra- for meningitis
            * IVP (polio)

Our immunizations are reviewed and made available as recommended by Medical practice guidelines.  We have the ability to order other immunizations on an as needed basis.
